This part should do what you want. Before ordering boards print out the pcb footprint at 1:1 scale and compare it to a real part to make sure the layout is correct.

Parts making in general is complex. For small numbers of parts you are likely better to ask. If you want to learn to make parts these two tutorials will tell you how (and apply to current Fritzing versions) or as usual turn you off from making parts. We do however need more people capable of making parts, they just aren’t easy to find.
I lately learned there aren’t links to the videos in Old_Grey’s tutorial so you need to do a google search for the title and then they come up on YouTube.
