This part should do what you want. Of note is the mounting holes in pcb are not drilled by default. If you want mounting holes you need to drag the core parts/pcb/hole in to the sketch, place it over the hole in silkscreen and set the size appropriately. As well only the pwm and ground connection on the 0.1in header pads appear in pcb.

No. Because the other three holes are a screw terminal and don’t have pads on the pcb (the 2 terminal screw terminal has extra pads in pcb.)
