These two parts should do what you want. For plugging the card in to 0.1in headers use the tht version of the part, to solder the part to a board use the smd version of the part. The difference is in pcb view (breadboard and schematic are identical)
the smd version only has pads on the top layer suitable for soldering to the board directly.

The web site in the .fzp file has the most complete pinout diagram that I was able to find on the net.

What Fritzing version are you using? The uploaded file works fine for me on Fritzing 1.0.1 on Win10:
