I am looking for the following part: Pololu - QTR-1RC Reflectance Sensor (2-Pack)
This part should do what you want. Note as usual in parts the mounting holes are not drilled by default. If you want mounting holes you need to drag the pcb->hole in to your sketch, place it over the hole in silkscreen and then set the size appropriately.
