You need to specify whether you want form C or form A contacts as the two are different.

Sorry for missed information. I need the Form C which is have 5 pin terminal.
This part should do what you want. Before ordering boards print the pcb footprint out at 1:1 scale (the default) and compare it to a real part. The holes are set to 0.055in (~1.4mm.)

Deleted as the pins appear to be backwards. See the corrected part below.

Hi everyone,
I have tested the component and seems like the position of NC and NO is wrong.
Here I attached new fixed one.

Seems odd, but the data sheet does not say what the alignment is so it is possible to be backwareds:
Note indication of top or bottom so I assumed the standard top view. If it is in fact bottom view (which it could be) the pins will be wrong. I will delete the original part to avoid confusion.
