This part should do what you want.

edit:

correct size of silkscreen

edit1:

correct the name of the schematic svg as it conflicts with a single gang schematic and probably causes the issue below.

from shaft center to the farthest pot pin is 16mm.

im sorry to report the part still has an issue. the schematic symbol shows the 2 pots. but the pot hidden in the body is unusable.

this is pcb of the pot above loaded in to Fritzing. It does not appear to be the pot above (which is likely the ALPHA-RV16AF which indeed does look like that but is single gang not dual.)
