I was struggling to get my hands onto a replacement Astrodyne MDCA-0108.
This is my solution:

It is slightly taller due to the carrier board.
It needs a 12V and a 5V PSU, the control pin is unused, which is not an issue as the device in question does not make use of this feature anyway. One or two SMD LEDs can be added but this is not required.
