This part should do what you want. Note pcb view has been suppressed as it connects via wires and thus won’t mount on a pcb. The breadboard image is fairly crude as there are no good pictures of the unit that I can find and the connections are shown from the top (the documentation shows them from the bottom!) but should match correctly if you remember that.
